The speaker places surgeons at a gurney beside a hell raiser and describes both through dedication, opposite tasks, hurt, and self annoited lives of trouble.

The poem sets 'the surgeons at the gurney' beside 'the hell raiser' who 'is there as well', both 'formed by their dedication / to twist the opposite / in outrageous task', and neither needing to go beyond 'the truth of hurt, / and self annoited / lives of trouble'.

Healer and troublemaker are leveled by the intensity of their commitment rather than its direction. The repeated 'Even as' keeps the comparison compressed and unsentimental.
